Question type Form

About the Form question type

The Form question type lets you present several input fields in a clear, structured row-by-row layout — ideal for situations where several pieces of information need to be captured compactly.

This makes it possible, for example, to capture personal information (such as name, address, contact details) or structured, related information (e.g. measurements, appointments, or key figures) at a glance.

Examples:

  • Personal contact details (name, email, phone number, date of birth)
  • Availability information (date, time, location)
  • Product specifications (weight, color, serial number)

Question and input field settings, value types

The settings for the Form question and its individual input fields largely match those of the "Single Line" question type.

For an overview — the value types supported by the "Form" question type:

  • Text – Free entry with no restrictions (default)
  • Number – Numeric entry only
  • Phone Number – Entry of a valid phone number
  • Password – Masked entry, e.g. for logins
  • Date (Calendar) – Selection via calendar widget
  • Time – Entry or selection of a time
  • Web address (URL) – e.g. https://www.example.com
  • E-mail – e.g. name@mail.com
  • Card Number – Only digits allowed; other characters are removed automatically
  • Pattern (regex) – Custom validation with regular expressions, e.g. for postal codes, license plates, IBAN, etc.
